Choctaw Nation Tribal Transit is designed to help those without adequate transportation to non-emergency medical appointments.

Choctaw Nation Tribal Transit cannot handle emergency transportation. In case of an emergency, please call 911.

When you call Choctaw Nation Transit, please provide the following information:

  • Your birthday
  • Your current Medicaid ID number
  • The time and day of your medical appointment
  • The name, address, and phone number of the health care facility you will be visiting
  • If you have special needs such as a wheel- chair lift
  • If you will need an attendant Drivers are not considered an attendant.

 

Passenger Eligibility and Procedures

 

  • Transit is open to anyone who lives in the 10 ½ counties of the Choctaw Nation District Boundaries, for non-emergency medical appointments.
  • All rides require advance notice of 5 business days before the scheduled appointment. (Saturday and Sunday are not considered a business day)
  • Transportation will be considered on a first come first serve basis / with elders, dialysis and chemo patients having priority.
  • All appointments need to be scheduled through the Dispatchers office
  • We can also assist with making arrangements with Sooner Ride for Medicaid Clients
  • Must be 18 years of age unless accompanied by and parent or legal guardian
  • Parents transporting children under 5 years of age must provide a child safety or booster seat
  • Drivers will call clients with pick up time the day before appointment between the hours of 4:00pm and 8:00pm (clients that are unable to be contacted or has not checked in with the dispatcher may not be picked up)
  • Please allow 15 minutes before and after scheduled pick up time for your driver.
  • Driver will wait at you’re designated pickup area for 5 minutes
  • Clients under the influence of drugs or alcohol will not be transported
  • Profanity nor harassment of drivers and/or other passengers will be tolerated
  • The bus will not leave until the last rider has finished their appointment
  • Wheelchair accessible van/buses are available
  • If a caregiver is required it is the responsibility of the rider to bring one. Drivers will help as much as possible but may not be able to stay with the rider throughout the appointment.
  • Attendant must be provided by the facility, for any client residing in a Home such as: Nursing Home, Women’s shelter, or Juvenile shelter (it may be a parent, guardian, counselor, or nurse).
  • Clients having day surgery will need an attendant (family or friend).
  • One family member may ride if room is available. You should always check before bringing them along.
  • Due to scheduling and limited drivers we may be unable to accommodate all continuous appointments such as chemo or dialysis.

No Show Policy

  • If under the influence this is a no show.
  • If driver shows for pick up and client refuses to go.
  • If you don’t call the day before to cancel.
  • After 3 NO SHOWS you may not be considered for a ride or voucher for up to 2 months.
