Are you open to the public?
Yes, we are open to the public. Anyone living in the 10 and 1/2 counties of the Choctaw Nation of Oklahoma are eligible, for nonemergency medical appointments.
How do I make an appointment?
All appointments must be made through the dispatcher's office.
What information do I need?
Your birthday, current Medicaid ID number, time and day of the appointment(s) [please note: all rides require 5 business days advance notice], the name, address, phone number of the facility you will be visiting, if you have special needs such as a wheelchair lift, if you need an attendant; Drivers are not considered attendants.
Are there age requirements?
Yes, you must be 18 years of age, unless accompanied by a parent or legal guardian.
Can I get a ride in an emergency?
Unfortunately no, in case of an emergency call 911.
My child is young, do I need to bring a safety seat?
Yes, you must provide a safety seat or booster seat for all children under 5 years of age.
Do any patients take priority?
Transportation is on a first come first serve basis, with the elderly, dialysis and chemo therapy patients receiving the highest priority.
Will I receive confirmation of my pick-up time?
Yes, your driver will call the day before the appointment between 1:00 PM and 5:00 PM. If you do not hear from your driver, please call the dispatcher's office.
